It is common for physicians and nurses to face ethical dilemmas every day while providing care to patients, especially those with critical illnesses. The situations where the answer can be “right” are very rare in such cases. 

This is especially more common during EOL (end-of-life) care, where the patient and caregivers might experience extreme emotions like grief, sadness, and loss. Oncologists often have to act as mediators through such difficult moments by advocating for their patients and connecting their caregivers with resources that would help them during the challenging phase.

During EOL care, the ethical dilemma may arise due to situations like patient's independence being compromised, communication breakdowns, lack of effective symptom management, shared decision making, and non-beneficial care. 

Physicians often have to navigate through such ethical dilemmas by suggesting the best possible treatment and advice while allowing patients and their family members the opportunity to manage EOL with courage and dignity.

Here, it is important for physicians to know when to consult an ethics committee at the hospital or institution.

They may consider reaching out to the ethics committee in the following situations.

  • Family members are not sure of whether or when to begin end-of-life treatment

  • You and the patient are distressed over the decisions

  • Religious and cultural beliefs conflict with the recommended treatment

  • The family or caregivers are not certain about who should make the decision in case the patient is unable to do so

  • No “right” answer and course of action seems to be available

Communication is the key to EOL care. Conversations with patients and their families about death and making the choice of EOL care can never be easy. Hence, doctors also need to improve their  communication techniques so that they feel more comfortable while having these difficult conversations with the patient and families.
